Occupational Therapist – Reablement Service
We are looking for a motivated and dynamic Occupational Therapist to join our Reablement Team. Successful candidate will need to have a flexible and holistic approach to meet the needs of our customers to promote their independence in the local community.
Reablement is an exciting fast paste service to facilitate hospital discharges and support people in the community to help them regain their independence.
You will be expected to contribute to the day to day management of the Reablement Service by working as an integrated member of the multi-disciplinary team within the community, usually in customers’ homes, to promote independence and choice for customers either by directly helping them to work towards their outcome focussed reablement goals as set out in their support plan or through the co-ordination and supervision of a team of Reablement Workers to deliver a high quality reablement service. Using person centred and Strength Based-approaches work closely with customers, carers, Reablement Officers and other professionals to meet the individuals’ needs improving discharges and transfer from hospital and helping to reducing prevention of admission.
You will need to demonstrate excellent assessment, supervision and support skills and be able to work collaboratively with health and social workers from across the teams.
Candidate must have a degree in Occupational Therapist and hold a current HCPC registration. It is essential the candidate is a car user and able to use the car for work purpose.
Experience in a community setting is desirable but not essential.
