Social Worker – Adult Transitions

The Adult Transition team are responsible for all initial referrals/assessments and implementation of support, as well as reviewing ongoing care and longer-term work including safeguarding enquiries. The teams support young people and adults from 16 to 25 years old who are living in the community with care and support needs and have an EHCP in place. We provide personal and practical support to help young people and adults to transition from children to adult services, promoting full and active lives, maintaining their independence and dignity, and having choice and control as far as they can to achieve their outcomes. To deliver Statutory Services to people who meet the eligibility criteria of the Adult Transition Team, by providing outcome focused assessments, and then facilitate the delivery of those identified services in order to achieve desired outcomes to meet the needs of the young person and/or adult.Please note that you need to be a fully qualified Social worker, experienced in Adult Social Work and transition.
