Transition Social Worker (SEND)

As part of the new Transitions Team, we have the opportunity to offer a permanent role as a Transition Social worker, sitting within the SEND Service in the Royal Borough of Windsor and Maidenhead.This role is one that may be of interest to someone with a background in Social care, working directly with young people who have SEND needs. This position offers an excellent opportunity for individuals with outstanding communication skills and a compassionate manner,  to work directly with young people and their families, putting the aspirations and outcomes of the young people at the heart of their transition to adult services. We encourage applications from candidates with experience in adult or children’s social care and those passionate about young people’s transition to adulthood.
