Throughout a student’s time on their placements, the necessary documents need to be completed to evidence their learning and progression. On this page you will find all the documents that need to be completed on the first and final placement.

Before each placement starts a pre-placement meeting should take place. To support you in making sure you and your student are ready to start please use the: Pre-Placement Meeting Guidance document.

If during either placement, a student, for whatever reason, is at risk of not successfully completing or passing their placement, and issues have not been resolved through supervision with their practice educator, practice supervisor, or through tutorials with their tutor please refer to the Placement Progress Support Needs & Concerns Report.

If a placement needs to come to an end, please use the Early Ending of Placement form.
